The FBI failed to investigate the reports which it received about a doctor who was eventually found guilty of abusing various gymnasts.
This was the conclusion of a 119-page report prepared by the Department of Justice Inspector General, about the case of Larry Nassar, the official doctor for the American gymnastics team.
Nassar is serving a lifetime sentence after being found guilty of sexually abusing various gymnasts.
Around 330 girls accused him of abusing them, including gymnasts who were considered to be the best in the country.
The report concludes that, despite the fact that the FBI had received the allegations, it had failed to seriously investigate them and two officers had lied when they were confronted with the mistakes they made.
The FBI admitted its shortcomings and described the report as one which discredits the organisation.
Rachel Denhollander, who was the first woman to accuse Nasser of the abuse, expressed her anger that while the FBI agents who failed to take action have now retired and are enjoying their pensions, the victims are still dealing with their trauma.
